Overview
The SOC (Start of Care) Nurse is responsible for providing comprehensive nursing assessments and initiating home care services for patients transitioning into care. The SOC Nurse conducts Start of Care visits, develops and implements individualized plans of care, coordinates multidisciplinary services, and ensures compliance with regulatory, agency, and payer requirements. The role requires strong clinical judgment, care coordination, documentation, and communication skills to support safe and effective patient outcomes. Works under moderate supervision.

What You Will Do
- Practice independently in the community as part of an interdisciplinary care team.
- Deliver personalized nursing and care management to patients in their home or care facilities.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
Qualifications
Licenses and Certifications:
BLS certification preferredCurrent New York State Registered Nurse license and registration required
Education
Associate's Degree in Nursing requiredBachelor's Degree in Nursing from an accredited nursing program preferred
Work Experience:
Minimum one year of recent clinical nursing experience in home care, medical-surgical, acute care, or related setting. requiredExperience completing OASIS assessments and Start of Care visits preferred. preferredDemonstrated knowledge of home care regulations and reimbursement practices preferred. preferredStrong computer and electronic medical record (EMR) skills required. requiredValid driver’s license or government-issued photo ID required based on operational needs. preferred
