As a Care Coordinator you are the Communication Hub, the Emotional Support Co-Worker, and the Air-Traffic Controller for our nurses and caregivers in flight. Coordinate daily caregiver schedules based on client care plans and staff availability

Support and communicate with caregivers working in the field

Complete client openings in the field

Respond to client and employee questions or concerns and assist with resolution

Maintain accurate documentation, records, and reports

Assist with quality initiatives, staff communication, and training support

Provide professional and courteous communication with clients, families, and team members

Must be at least 18 years of age

High school diploma or equivalent required

Ability to multitask and stay organized in a fast-paced environment
