St. Elizabeth Healthcare is hiring a Registered Nurse Case Manager for the Hospice unit located at the Edgewood, Kentucky location.

Shift: 8:00 am - 4:30 pm; Monday - Friday.

Hours: Full-time, 40 hours weekly.

The RN Case Manager is responsible for provision of quality care to all patients and the achievement of excellent patient outcomes. The RN Case Manager assesses patient needs, assists in the formulation and implements the physician’s plan of care to reach achievable goals mutually agreeable to the patient and care providers. These activities are carried out in a collaborative manner that promotes quality, continuity and safety.
