As a Home Health Physical Therapist, you'll enjoy the autonomy of managing your schedule, providing one-on-one care in patients' homes, and helping individuals regain strength, mobility, and independence where they are most comfortable.

• Provide skilled physical therapy services in accordance with physician orders and individualized plans of care. • Evaluate patients and develop personalized treatment programs to improve function and quality of life. • Assess mobility, strength, balance, and safety needs. • Document patient evaluations, treatments, and progress in a timely and accurate manner. • Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes. • Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals. • Supervise Physical Therapist Assistants as appropriate.

• Graduate of an accredited Physical Therapy program. • Current Physical Therapist license in Pennsylvania. • Current CPR certification (or ability to obtain upon hire). • Valid driver's license, reliable transportation, and proof of auto insurance. • Strong communication, organizational, and documentation skills. • Home health experience preferred, but not required.
